Appointment

To start the appointment process, fill out the Appointment Registration Form below. Fill out the “agent” form if you’re registering for yourself, or the “agency” form if you’re registering as an agency managing multiple brokers. In order to apply for appointment with Friday Health Plans, please be advised that the Agency and Agent(s) must have unique email addresses. We are unable to process applications that utilize a shared email address across multiple accounts.

Within 48 hours of submitting the registration form, you will receive an email with a secure URL to the Friday Broker Contract Portal (Note: the email will come from Evolve). Follow the Broker Onboarding Instructions, complete the contract, and upload your credentials. In order to apply for appointment with Friday Health Plans, please be advised that the Agency and Agent(s) must have unique email addresses. We are unable to process applications that utilize a shared email address across multiple accounts. We will email your approval within a week or follow up if we need additional information.
